Please bring your insurance card and a picture ID with you so that we can make a copy for our records.

Anytime that you have a change in insurance, address or phone number, be sure to notify the receptionist so that the change can be entered into our system.

Filing Insurance Claims

Orlando Family Medicine will provide insurance claim filing for the insurance plans that we participate. We will file PPO’s that are out of network at a higher out-of-pocket expense. If there is a balance due after payment is rendered by the insurance company, we will expect prompt payment of that balance by the patient or responsible party.

It is the patient’s responsibility to provide Orlando Family Medicine with current insurance plan information prior to services rendered in order for accurate billing of services to be filed.  At times, we may require the patient’s intervention to assist us and we appreciate the patient working with us in this regard.

Remember: Payment is due at the time of service. We do accept cash, checks and credit cards for payment, such as VISA, MASTERCARD and DISCOVER.
